Informed Consent

Informed Consent Please read, and check box below when submitting this informed consent for your appointment. I am requesting a session for myself and I am 18 years or older. I understand that all information is offered for personal and educational purposes, and is not intended to be used in lieu of any medical, psychological, legal or financial counsel, or minimize any recommended services by other professionals. The information is strictly open to interpretation by the client. I understand my rights to exercise free will and I will not hold Linda McCarthy Ph. D responsible for any actions or decisions I make as a result of the information I receive during a session. I understand I am not being offered any guarantees by Linda M. McCarthy Ph.D. Each session is my perception of the conversations between Linda M McCarthy Ph.D and myself and mine alone. I understand that I have the right to end the session, for a full refund within the first 10 minutes, if I am not satisfied with the direction of the conversation. I agree to provide 24 hours notice of cancellation to avoid a possible cancellation fee, which is 50% of the amount of the session. I have read and understand the nature of the services, length of session, and fee structure based on the information provided under " Service Provided" on the New Direction Coaching website. I understand the information shared in the session is strictly confidential, and my information will never be shared with a third party.
